I understand that participation in the Los Angeles County Metropolitan Transportation Authority Metro Crenshaw/LAX Transit Project Business Solution Center(BSC) is voluntary. I agree that my best efforts will be made to continue my business in its current location. I further agree to cooperate in completing surveys designed to evaluate BSC Services. I authorize the BSC to receive and accept relevant information through its assigned case manager and/or business referral manager regarding my business, expect that information to be held in strict confidence by him/her, and the BSC.
By my signature below, I certify that any and all information and records submitted to the BSC are true and a correct representation of my business circumstances. I further understand that there are no guarantees or assurances with recieving BSC services.
